By filling in this form Parents/Guardians consent to the participation of the child / children named on this form in all rugby and related activities including photographs and videos and in the absence of a parent/guardian, to any emergency medical treatment deemed necessary. Any medical conditions should be listed on the reverse of this form and brought to the attention of the relevant coach and team manager.

It is the responsibility of ALL PLAYERS or GUARDIANS OF YOUTH and MINI PLAYERS to ensure that they are medically fit to play rugby and exercise and to ensure they have their own participation sport insurance cover!
