I confirm that I have parental responsibility for the above player(s) and that their details are correct to the best of my knowledge. I consent to the player(s) participating in the various club activities throughout the season.
In the event of injury or illness, having parental responsibility, I give permission for medical treatment to be administered where considered necessary by a nominated First Aider, or by suitably qualified medical practitioners. If I cannot be contacted and my child needs emergency hospital treatment, I authorise a qualified medical practitioner to provide emergency treatment / medication.
I permit the use of photographs and video recordings taken by K.U.F.C Staff of my child(ren) /Club member to be used respectfully as part of club activities. I understand that these photographs/Recordings could be used by the club for the administration of awards and club promotion, these photographs/recordings will not be used for any other purpose.
All juvenile players must report to their correct age group if they wish to be considered for Galway Development Squads (Elite Player Development)

I confirm that I understand in relation to all adult player the club Only carries the minimum liability cover required by the Galway District League.
Juvienule players are covered by Additional players insurance full details available on request to the club Secretary
I confirm that I understand that I will be liable to pay 100% of Excess for any insurance claim. K.U.F.C. Will then reimburse 50% of total excess following successful completion of insurance cover payout to cover said claim.
I understand that an application to any particular squad does not Guarantee entry to the squad requested, although every effort will be made to accommodate where possible. In the event squad request is unsuccessful this does not give grounds for membership fee to be refunded.
I Understand that most team communication is performed using instant messaging platform “whatsapp” free-of-charge, i will download to my phone where possible and I will respond when asked to do so.
Correct D.O.B Must given at all times during the registration process with K.U.F.C.. in the event the D.O.B given is incorrect the club may terminate club membership and will result in forfeiture of fees paid.

I understand that if i opt to pay by instalments i am committing to pay full annual membership fee as set out by K.U.F.C. regardless of the players attendance either by choice, eligibility or any for other reason, at training or matches in the season to which the membership relates.
If any instalment fails to be collected on due date the remainder of all moneys due under said membership becomes payable immediately and must be paid in full within 14 Unless an alternative payment plan agreed by the committee. Any such agreement must be given in writing.
I understand that no new membership can be accepted by the club where monies remain outstanding. In such cases where money is owed from a previous membership by any individual, the FAI/club registration process will be placed on hold until such time account is brought up to date.
For adult players:
I confirm that I understand that no players insurance is currently or will be put in place for the Coming season and Kinvara United Carries the minimum level of insurance required by Galway Football Association. Any Injuries sustained as a result of involvement in participation in sport, with the exception of cases where negligence is proven are not covered.
